Order Form
INSTRUCTIONS/INFORMATION: Complete a separate form for each product you are ordering. Payment is by credit card (via PayPal, see their website for more information at www.paypal.com), personal check or money order, if drawn on a US bank. Once your order and photos are received, you will be emailed a private link to view your proof online within five business days. You will then review your proof, make any changes, and complete your order.
		I will be sending a digital or scanned photo. I will be sending a regular photograph. Check this box if you wish to pre-order envelopes. They will be shipped the following day at no extra charge. Check this box to give Presents of Mind permission to use your image for advertising on our website or in print.
· Negatives and Polaroids cannot be used. · Prints are always handled with care and shipped back to you in original condition with your order. Do not send prints larger than 8x10. · Professionally taken photos are required to be accompanied by a signed copyright release form. Your photographer may sign our generic release, or use their own. Professionally taken photos submitted to Presents of Mind without a signed release will not be used. · Email your digital camera images to orders@presentsofmind.com. Please reference your name, total number of images you are emailing, and also state if your order form will arrive via mail or fax.
